What is Hospice Care?

Hospice is patient centered care provided in the home setting when you might be facing a terminal diagnosis. The hospice team comes to you anywhere that you call home, whether that be in your house, an assisted living community, skilled nursing facility, or board and care residence. We are there to provide those services for you and prioritize your comfort.

This high level of outpatient excellence in care is achieved by bringing together a team of passionate and compassionate professionals with the same goal in mind: your comfort. The team consists of physicians, nurses, pharmacists, home health aides, spiritual counselors, social workers, volunteers, and more.

Patients receive regularly scheduled visits. In addition, you can call us 24 hours a day, seven days a week, and we will be available to address any concerns. You can think of us as comprehensive medical support brought directly to you.

Medications, Supplies, and Equipment

The biggest tools we have are medications, and we use these to make sure that you are comfortable. In addition, we provide supplies, equipment, and procedures that can be performed in the home setting. The goal is to help you avoid hospitalization whenever possible.

This program is covered by most insurances, including Medicare, Medi-Cal, and most private insurance plans. You can think of hospice as compassionate support at a really difficult time. We are here for you to make sure that this journey is as comfortable as possible.
